1. Your eye examination prescription is given to you following your eye examination. It contains the outcome of the eyes test. This includes the prescription/numbers relating to the strength of the lenses to correct your eyesight for seeing better in the distance, such as watching TV or driving or near such as reading a book.
2. It may have comments about your level of vision (V) and visual acuity (VA).
3. It may have comments about the health of your eyes.
4. It states when you are next due for your eye examination.
5. The name, registration number of the Optometrist and address of the practice you had your eye test.
6. The table below illustrates the format of the prescription.
